What they offer: Provides assistance to active and retired firefighters, their families and others not necessarily associated with the Benevolent Fund who, became of unforeseen circumstances are in need of assistance. Its mission includes providing a memorial tribute. Programs include providing flowers and donations for fallen firefighters and family members, as well as assistance for active and retired firefighters.

What is offered: Perform a DNA test that detects abnormal esophageal cells, such as Barrett's Esophagus, precancerous cells with dysplasia and cancer.
Requirement: NO Food or Drinks (2) two hours prior to exam, bring ID/Insurance cards.
If you would like to be screened for esophageal pre-cancer/cancer and have three (3) of the risk factors listed below,
Chronic acid reflux/heart burn or GERD symptoms
o Male
o >50 years old
o White race
o Tobacco smoking
Obesity
Family history of Barrett’s Esophagus or Esophageal Adenocarcinoma (cancer) in a first-degree relative
Occupational or environmental exposure (smoke inhalation/fire/chemicals)

Any other important information: They have partnered with Upscripts, a NO COST individualized telemedicine service (and if candidate is pre-approved for event, formal order will be issued before event to reduces wait-time). They also notify the patient of their results from board-certified physicians and send certified letters in mail.
ESOGUARD AND INSURANCE
Lucid Diagnostics accepts all insurance plans and directly bills your medical insurance carrier for the EsoGuard test. Their team is focused on gaining EsoGuard coverage.
What to expect
01 Lucid Diagnostics Notification
Lucid Diagnostics will send you a notification that a claim has been submitted to your insurance provider with a consent form **optional to complete** that allows Lucid Dignostics to appeal on your behalf if your insurance provider denies the claim.
02 Claim Submission
Lucid Diagnostics will submit a claim for the EsoGuard DNA test to your insurance provider directly.
03 Explanation of Benefits
Once your claim has been processed, your insurance provider will send you an Explanation of Benefits (EOB) which explains whether the provider has approved or denied the claim. NOTE: THIS IS NOT A BILL.
04 Patient Portion
If your insurance provider denies the claim, Lucid Diagnostics will work on your behalf and appeal the denial. If the claim is denied after appeals, Lucid Diagnostics considers it adjudicated, and NO further patient follow-up occurs.
If your claim is approved by your insurance provider, there MAY be patient out-of-pocket responsibility due to co-insurance or deductible requirements set by your insurance provider. Lucid Diagnostics will bill you for the patient responsibility portion, as is required by law.
• CURRENTLY, they do NOT refer unpaid claims to a collection agency. They offer various financial assistance programs for patients. If you receive a bill, please contact their team at 1-888-LucidDx for further assistance before paying anything.

What they offer: To assist firefighters and EMS providers and their immediate families diagnosed with cancer by providing badge to badge support, training and guidance.

What they offer: Primary and specialty care including Care Navigation, Diabetes Care, Endocrinology, Health and Nutritional Coaching, Integrated Behavioral Health Services, and Physical Therapy. All 3 wellness centers are available to members of The Fund and their families free of charge.
Any other important information: Will offer Wellness Vouchers! Health by Design offers two tests that can screen for as many as 50 different types of cancer. Both tests require a simple blood draw and your physician will review your results or contact you sooner if any abnormalities need to be discussed.
Hereditary Cancer Test: My Risk is a one-time hereditary test to evaluate 48 genes associated with 11 different types of cancers. This test used in combination with familial history to offer personalized guidance for patients and providers to work together to reduce risk of developing cancer or detect it at an earlier, more treatable stage. Multi-Cancer Early Detection Test: Galleri is a multi-cancer early detection test to identify active cancer signals across more than 50 types for select patients with elevated risks. If a cancer signal is detected, the results point to where in the body the cancer signal is coming from. What they offer: California Professional Firefighters’ PERSONAL EXPOSURE REPORTING (PER) APP offers an easy, secure, and comprehensive method of recording your exposure to cancer-causing chemicals and biological toxins. These reports provide critical documentation needed in Workers’ Compensation cases involving job-related illnesses and in medical diagnosis for those illnesses. Using the confidential PER website, you can keep a private record of: Incident information, Nature and duration of exposure, medical symptoms experienced, medical diagnosis and treatment, and Recurring exposures on the job (diesel fuel, etc.). Best of all, reporting is secure. You create your own name and password, and the information is encrypted for your protection. PER is YOUR record. Nobody can access your records except you. The records you create are yours forever ... even after you retire.

They are recruiting licensed firefighters who have completed at least five years of service with their assigned fire stations and are currently on active duty. What they offer: The program that we believe to be best deployed for the early detection of cardiovascular disease and cancer is the UDS ultrasound-based tests. This is a gold-standard comprehensive program that consists of 9 body parts and organs:
